Our Mission at the Holyoke Health Center is to "Improve the health of our patients through affordable, quality health care and comprehensive community-based programs to create a healthy community."
At Holyoke Health we offer comprehensive medical services by board certified physicians and Nurse Practitioners. We deliver care in a Patient Centered Medical Home model where you and your primary care provider lead your individual health team. Your dedicated health team is made up of your primary care provider, nurse, nutritionist and other specialists as needed to work together with you to manage your health, injuries, illnesses and chronic diseases. This model allows you, the patient, to be in charge of setting your own goals with the support of your health team.
Our primary care services include preventative care, routine checkups, immunizations, and the management of acute and chronic illnesses. We have doctors, nurses, and Community Health Workers who focus on helping you manage your chronic diseases such as; asthma, diabetes, hypertension, cardiovascular disease, depression, and HIV/AIDS. Our diabetes self-management education class series is a comprehensive patient centered approach to empower people living with diabetes to be an expert on their health and chronic disease. This innovative program involves a Community Health Worker, Nurse, and Registered Dietician who work in conjunction with your primary care provider.
Our Pediatric Department offers comprehensive care for your child by board certified pediatricians. We offer compassionate care that brings the entire family into the health team for the child’s health needs. We have pediatricians, Family Providers, nurses, and Community Health Workers who work together for you and your family to manage the health of your child.
We have an onsite lab where you can get your blood drawn and avoid needing to go to a separate lab.
Same Day Care is available for you if you have an injury or illness that needs immediate care but is not severe enough to go to the hospital emergency room. This is not for life-threatening emergencies such as chest pain or difficulty breathing. For these types of emergencies call 911. We have a triage nurse available 24 hours a day to assist Health Center patients if you or a family member need medical advice by calling 413-420-2200.
