Prevention & Wellness Trust Fund

Holyoke Health is the lead organization recipient of the Prevention and Wellness Trust Fund grant.  Awarded in March of 2014, the Prevention and Wellness Trust Fund (PWTF), is a unique program that was awarded to nine community-based partnerships in the state to help fight chronic illness and to improve health outcomes while reducing health care costs.  Holyoke Health leads the Holyoke collaborative of the PWTF which includes these partner organizations:

This group works together to address the following health issues:

  • pediatric asthma
  • smoking cessation
  • hypertension
  • obesity
  • oral health

The collaborative members work together to create community-specific programs which address these health issues and aides patients in getting wrap around support.
