Community Programs

Holyoke Health has seven community programs in operation at this time.  These programs are created to meet the needs of the community that we serve at the health center and are staffed by a medical provider, Community Health Workers, and patient educators and outreach specialists.  These community programs address specific health issues that are present in our communities, and address specific populations who are in our communities,  in a holistic, community centered, and collaborate manner.
