Ryan White Program
Holyoke Health Center is a recipient of the federal Ryan White HIV/AIDS Part C Early Intervention Services grant. The Ryan White program provides patients living with HIV with access to infectious disease providers, medical case management, treatment adherence services, care coordination, and benefits coordinators who work to connect patients to community resources for food insecurity, housing insecurity, transportation assistance, and prescription payment assistance.

Case Management
Case management services for patients living with HIV are provided through our partnership with River Valley Counseling Center. Case managers assist patients with complex health problems, such as HIV, with understanding their diagnosis and conditions, and providing access to community resources to improve the patient's overall health.

Holyoke Health Center provides medical, dental, and pharmacy services to all of our patients, regardless of your ability to pay for them. If you do not have health insurance, you can apply for health insurance with one of our Certified Application Counselors and you may be eligible to receive services on our sliding fee scale.
