HHC, Inc. Background:
Holyoke Health Center, Inc. (HHC) is a federally qualified, non-profit community health center (FQHC), in Holyoke, MA with a second location in Chicopee, MA. HHC is a one-stop shop for healthcare, providing a wide range of services all under one roof, including:

With over 350 employees, including medical and dental providers, pharmacists, nurses, medical and dental assistants, case managers, community outreach workers, financial counselors, and more, HHC serves over 20,000 patients annually. HHC has an on-site pharmacy at each location in Holyoke and Chicopee. The pharmacies are staffed with 14 full-time Pharmacists, Pharmacy Residents, 1 Adjunct Pharmacy Professor, 11 Certified Technicians, 10 Clerks and 2 Community Health Workers.

Pharmacy
With a pharmacy location in Holyoke and in Chicopee, HHC provides pharmacy services to all patients in the community; 340B access for patients with HHC primary care and retail access for patients with primary care services outside of the HHC medical home. The on-site community pharmacy, fills approximately 6,000 prescriptions per week, is owned and operated by HHC and is an integral part of all services offered in the HHC medical home. The pharmacies accept many insurances including MassHealth, MCOs, Medicare, CCA, MedicareD and Health Safety Net. There are insurance enrollment services available within the pharmacy in order to assist patients with their insurance needs. Our pharmacy also provides clinical pharmacy services, where pharmacists are integrated into our provider team. We provide direct patient care, including utilization of CDTM agreements.
