Holyoke Health

Holyoke Health is a Federally Qualified Community Health Center located in the beautiful Pioneer Valley of Western Massachusetts.  While our main community health center site is located in downtown Holyoke, Holyoke Health has additional sites in the neighboring city of Chicopee, and we have dental sites located around Hampden County.

For over 40 years, Holyoke Health has provided medical, dental, clinical and dispensing pharmacy and community programming services to the greater Holyoke community.  Designated as a Medically Underserved Area, caring for some of the most vulnerable members of the community, Holyoke Health is a crucial safety net for patients in the area.  Holyoke Health is the only free standing community health center in the greater Holyoke area and is committed to providing the best and recently-researched models of care.  We are recruiting advanced practitioners for our medical, dental and pharmacy departments.

Holyoke Health is located in one of the most beautiful valleys in the state with easy access to large urban cities, mountains, and coastal destinations, we are uniquely situated to provide a wealth of academic opportunities, outdoor recreation and art and cultural happenings.

Holyoke Health employs more than 55 advanced practitioners in two primary care and speciality sites and four satellite sites.  We provide pediatric, primary and family medical care, same day urgent care, chronic disease care and self-management guidance, HIV Case Management, Substance Abuse Programming, Medication Therapy Management and pharmacy care, as well as pediatric and adult dental, oral surgery and orthodontics.  Our services are supported by innovative community-based programs that revolve around community engagement and education about health conditions.

Continuing Growth and Advancement

As Holyoke Health continues to grow, we also continue to strive to improve our quality of care, access to care and services, and to consistently meet the unique population needs of the greater Holyoke and Chicopee communities. Holyoke Health started in the basement of a tiny building forty years ago and since grown to include two full service health centers with on site pharmacies and dental departments, four stand-alone dental sites, and several community, organizational and governmental partnerships. We have completely integrated behavioral health services into our primary care medical home and are continually looking for innovative ways to provide comprehensive health care services for our patients.

Holyoke Health's advanced practice providers, nurses, medical assistants, pharmacists, and community health workers participate in serving specific patient populations that interest them by sitting on committees and focusing on specific community engagement. One example includes the Women's Health Committee. Holyoke Health values inter-professional teamwork and the importance of working to create opportunities for our health care professionals to focus on personal interests, medical conditions, and barriers to care that are specific to our client population.

Technology Supporting Quality Care

Holyoke Health uses the latest technology to enhance quality, safety and efficiency of care.  We have a fully integrated Electronic Medical Records (EMR) system which increases continuity of care for our patients across all departments.  We are an active participant in the state's unique Massachusetts Health Information Highway (HIway) referral system that links area providers (including the local hospital Holyoke Medical Center, behavioral health providers and other community organizations) to enable real time sharing of patient health information. Holyoke Health is also a level 3 Patient Centered Medical Home and is consistently pursuing all opportunities to stay up to date on this model.
