Holyoke Health's two locations, Holyoke Health Center and Chicopee Health Center, offer primary care and pharmacy services for both pediatric and adult patients creating a multitude of opportunities to participate in more specialized community and clinical initiatives. Provider Champions spearhead projects to improve population health as well as lead community involved committees and programs that interest them such as breastfeeding resources, young adult sexual health, and both pediatric and adult healthy weight initiatives.

Our pediatric, family, and adult medicine departments provide comprehensive medical care to families and individuals of all ages and income brackets. We have a state-of-the-art teaching kitchen that is used for our Diabetes Self-Management Classes which combine goal setting, chronic disease education, and diabetes support. Our groundbreaking model of Shared Appointments reflect how we have successfully integrated behavioral health and nutrition in to well-visits and take a whole team approach to treating a variety of health issues affecting our patient population.
