Our Philosophy, what is a Patient Centered Medical Home:

A Patient Centered Medical Home is a system of healthcare delivery where medical providers work as a team to provide the best care for each individual patient. Our approach is comprehensive, coordinated, and focused on quality, safety and the best outcomes for each patient. We believe that a patient centered medical home is the best way to care for members of our community from the least complicated to the most complex conditions. As our patient, you are part of the team of highly qualified health professionals working to improve and maintain your and your family’s health.

At Holyoke Health, our goal is to see each patient as an entire person. When appropriate, we invite family members to participate in the patients’ health-team and be involved in the individuals’ health plan. With respect to our community, we provide all services in English and Spanish to ensure that each patient understands their health and health plans fully. Our compassionate medical providers are experts in their fields, who believe that everyone deserves excellent medical care with access to the latest technology and facilities, whether a patient has private health insurance or no insurance at all.
